Baby Blues!

Baby is here! Just when you think you should be on top of the
world, you find yourself feeling blue, teary and depressed. How come
your ‘bundle of joy' is making you so unhappy?
"Pregnancy and birth are stressful events for any woman,"
explains Dr. Nor Hamidah Mohd Salleh, a Consultant Psychiatrist and
mother to six children. She adds, "After the delivery, most women are
exhausted physically, mentally and emotionally. All these changes will
have an impact on a new mother."
Most new mothers will experience ‘the blues' after having
their babies, as their hormones struggle to cope with the body changes.
On top of the fatigue and pain from stitches, there are also the added
responsibilities of caring for the new baby, which includes sleepless
nights and coping with baby's crying.
I've got the blues…
"Post-partum blues are particularly common among first-time
mothers, with one out of two mothers suffering from it," notes Dr. Nor
Hamidah. Normally, they will snap out of it within a few days or a week,
given sufficient rest and support from their social network such as
their spouses, mothers, in-laws or friends.
The severity of the condition depends on several factors, which includes:
- Whether the pregnancy is planned
- The health condition of the mother during pregnancy
- Whether the birth experience was unpleasant
- Whether there is any support and help from friends and family
- The relationship with the spouse at time of pregnancy and birth
Dr. Nor Hamidah explains, "Women who have less conflicts
within their family network are less likely to experience the blues or
recover from them faster, whereas women who have to cope with their new
baby alone and far away from family members may start feeling depressed
from their fatigue and isolation."
Having an understanding spouse who gives emotional and
physical support during night awakenings, breastfeeding problems and
colic can also help new mothers cope with the blues.
Blues or post-partum depression?
Although the symptoms are almost similar, post-partum
depression can be more serious and have more far-reaching effects on the
baby and mother. The most notable difference is that the blues usually
lasts less than a week, whereas post-partum depression lasts more than
two weeks and can drag on for months.
For both conditions, the symptoms start appearing around two
weeks after delivery of baby. Dr. Nor Hamidah stresses that post-partum
depression is a medical disorder and not just an emotional swing as it
can have severe repercussions such as suicide or psychosis if left
untreated.
"Post-partum depression is not very common, with about one or
two out of 1000 mothers suffering from it," she says, citing symptoms
such as insomnia, disillusionment and obsessiveness. They also become
hyper-sensitive to criticism and start imagining things such as hearing
voices in their heads.
"They may feel intense hatred for the child or themselves for
particular reasons, such as getting a girl when they want a boy, losing
their sexy figure or their career. These dangerous feelings need to be
controlled so that they do not harm the baby or themselves," she points
out.
What is important is to recognise post-partum depression as a
real problem, so that mothers who need help are not just being brushed
off as ‘having the blues'.
Treatment for post-partum depression
"Most patients who are experiencing post-partum depression
will first be tested for medical disorders such as hypertension, which
may be a cause for their depression. Once these diseases are ruled out,
then they will be treated according to the severity of their
conditions," says Dr. Nor Hamidah.
Treatment can be in the form of counseling, medications such
as sedatives (to help them relax and sleep), anti-depressants (to
control depressive feelings) or Electro Convulsive Therapy for severe
cases. As women who have post-natal depression are usually those with
pre-existing mental problems, they would be advised before having
children so that they can make the necessary preparations to avoid
unwanted repercussions.
"Whether it's the blues or post-natal depression, most
would-be parents must make the right choices before the baby comes, such
as having a confinement lady or someone to help the new mother cope,
and agreeing on the number of children a couple wants. Research shows
that 70% of babies are unplanned pregnancies, so obviously there are a
lot under pressure from having more children than they wanted!
"The best way to avoid any sort of blues is to build a happy
marriage and equip yourself with parenting skills so that you don't feel
helpless with the new baby," concludes Dr. Nor Hamidah.
